4. Food Safety Management System Plan

5. Layout Plan of the Premises

8. Proof of Incorporation
For companies, a copy of the Certificate of Incorporation issued by the Registrar of Companies (RoC) must be submitted along with the FSSAI registration application.
9. Source of Water
Details of the source of water used in the manufacturing or processing of food products along with a water testing report must be provided to ensure the safety and quality of the food products.
10. Declaration Form
The applicant must sign a declaration form affirming the accuracy of the information provided in the FSSAI registration application and agreeing to comply with the food safety regulations.
